CHC was developed for Pennsylvanians who are eligible for Medicare and Medicaid, or eligible for Medicaid-funded LTSS. This new initiative is a managed care program by the departments of Human Services (DHS) and Aging (PDA) to: (1) enhance access to and improve coordination of physical health care and; (2) create a person-driven, long-term support system in which people have choice, control, and access to quality services that provide independence, health, and quality of life.
