Doctors who have transformed their practices to “patient-centered medical homes” have improved patient care while providing savings.
According to an analysis by Blue Cross Blue Shield of Michigan, doctors that adopted the “medical homes” practice model, which involves evening and weekend office hours, 24-hour-a-day phone access to a care team, and an emphasis on preventive care and proactively managing chronic disease, have saved an estimated $155 million.
The savings estimate is based on prevented claims for care in the first three years of the program that began in 2009.
BCBSM senior vice president Doctor David Share says in the medical home model, it is easier for patients to see or consult their physician. He says “If you can get them in right away, you can get the condition under control and keep them out of trouble – and out of the inpatient setting and the emergency department setting.”
The medical home program involves 1 million Blue Cross members and 2 million Michigan residents – because the physicians follow the approach for all their patients.
